Delegation (TAPE)

The acronym for tasks that ONLY the RN can perform

Definition

TAPE is a memory aid for tasks that can ONLY be performed by a Registered Nurse (RN), these cannot be delegated to LPNs or unlicensed assistive personnel (UAPs):

  • T: Teaching
  • A: Assessment (initial)
  • P: Planning
  • E: Evaluation

Five Rights of Delegation

  1. Right task
  2. Right circumstance
  3. Right person
  4. Right direction/communication
  5. Right supervision/evaluation

Scope of Practice

  • UAP: ADLs, I&Os, vital signs on stable patients, feeding, bathing, ambulating
  • LPN/LVN: Sterile procedures, stable patient medications (not IV push), wound care, stable focused assessments
  • RN: Initial assessment, unstable patients, care planning, patient teaching, evaluating outcomes, IV push medications (most), blood transfusion administration

Delegation questions are abundant. Remember: RN retains accountability.
