The acronym for tasks that ONLY the RN can perform
Definition
TAPE is a memory aid for tasks that can ONLY be performed by a Registered Nurse (RN), these cannot be delegated to LPNs or unlicensed assistive personnel (UAPs):

- A: Assessment (initial)
- P: Planning
- E: Evaluation

- Right task
- Right circumstance
- Right person
- Right direction/communication
- Right supervision/evaluation
Scope of Practice
- UAP: ADLs, I&Os, vital signs on stable patients, feeding, bathing, ambulating
- LPN/LVN: Sterile procedures, stable patient medications (not IV push), wound care, stable focused assessments
- RN: Initial assessment, unstable patients, care planning, patient teaching, evaluating outcomes, IV push medications (most), blood transfusion administration
