Disorientation

Confusion regarding person, place, time, or situation

Definition

Disorientation is an impairment in cognition where a person cannot correctly identify person (self/others), place (location), time (day, date, year), or situation. It is a cardinal sign of altered mental status.

Orientation Levels

  • Oriented × 4: person, place, time, situation
  • Oriented × 3: person, place, time
  • Oriented × 2: person, place
  • Oriented × 1: person (often last to be lost)

Common Causes

Hypoxia, hypoglycemia, sepsis (especially UTI in elderly), dehydration, electrolyte imbalance, medication effects, substance use, head trauma, stroke, dementia, delirium, post-anesthesia.

Nursing Interventions

Assess ABCs and oxygenation first. Obtain point-of-care blood glucose. Identify and treat underlying cause. Use reorientation techniques (clock, calendar, familiar items), minimize noise and sleep disruption, maintain safe environment. Avoid restraints when possible.
