Least Invasive First

Attempt non-invasive treatments before invasive interventions

Definition

Least Invasive First is a principle that recommends attempting non-invasive or minimally invasive interventions before moving to more invasive ones, when clinically appropriate.

Examples

  • Repositioning before administering oxygen for mild dyspnea
  • Nonpharmacologic pain management (ice, repositioning) before opioids
  • Oral rehydration before IV fluids
  • Dietary changes before medications
  • Physical therapy before surgery

Exception

In emergencies, move directly to the most effective intervention. Do not delay life-saving care (e.g., IV fluids for severe dehydration, not oral).
