Matrix Multiple Response

Definition

Matrix Multiple Response is an NGN grid item where the student may select multiple checkboxes per row, per column, or across the entire grid. It tests the most complex clinical reasoning and is often used to compare and differentiate interventions across multiple dimensions.

Example Structure

Rows: Nursing interventions. Columns: Indicated, Contraindicated, Monitor for effectiveness. The student may check multiple cells per row because an intervention might be indicated AND require monitoring.

Scoring

Uses partial credit with plus-minus scoring on a per-cell basis. Each correct selection earns a point; each incorrect selection loses a point. Minimum score per item is zero. This encourages precision over guessing.

Strategy

  • Slow down. Do not rush through rows.
  • Treat each cell as an independent yes/no decision.
  • Re-read the clinical context before each row.
  • Apply clinical reasoning: what do you know about the intervention and the patient?
  • Only select cells you are confident about.

NCLEX Relevance

One of the most complex NGN formats. Frequently appears in case studies for medications, interventions, and patient education scenarios. Mastery requires integrating pathophysiology, pharmacology, and nursing priorities.
