Pyrexia
Elevated body temperature; fever
Definition
Pyrexia is an elevation of body temperature above normal, generally greater than 100.4°F (38°C) orally. It is the body's response to infection, inflammation, malignancy, or medications.
Fever Patterns
- Intermittent: Spikes with return to normal.
- Remittent: Fluctuates but never normal.
- Sustained: Persistent elevation.
- Relapsing: Alternating febrile/afebrile periods.
Nursing Interventions
- Antipyretics (acetaminophen, ibuprofen)
- Cool environment, lightweight clothing
- Tepid sponge bath (not cold; causes shivering)
- Encourage fluids
- Monitor temperature, VS, and mental status
- Cultures as ordered (before antibiotics)
- NEUTROPENIC FEVER = emergency
Pediatric Considerations
No aspirin (Reye syndrome). Seizure precautions for febrile seizures. Educate parents on appropriate antipyretic dosing.
NCLEX Relevance
Neutropenic fever is an emergency. Aspirin contraindicated in children.
