Stability vs. Instability

A prioritization framework: see the unstable patient first

Definition

Stability vs. Instability is a prioritization framework used on the NCLEX: patients with stable (expected) findings are lower priority than those with unstable (unexpected) findings. Unstable patients are at higher risk of deterioration and require urgent assessment.

How to Apply

  • Stable = expected findings consistent with diagnosis
  • Unstable = new, worsening, or unexpected findings
  • See unstable patients first

Examples

  • CHF patient with pink frothy sputum (NEW, unstable) BEFORE COPD patient with baseline dyspnea
  • Post-op patient with increasing restlessness (unexpected/unstable) BEFORE patient with baseline anxiety

NCLEX Relevance

Core strategy for 'Which patient do you see first?' questions.
