Stability vs. Instability
A prioritization framework: see the unstable patient first
Definition
Stability vs. Instability is a prioritization framework used on the NCLEX: patients with stable (expected) findings are lower priority than those with unstable (unexpected) findings. Unstable patients are at higher risk of deterioration and require urgent assessment.
How to Apply
- Stable = expected findings consistent with diagnosis
- Unstable = new, worsening, or unexpected findings
- See unstable patients first

- CHF patient with pink frothy sputum (NEW, unstable) BEFORE COPD patient with baseline dyspnea
- Post-op patient with increasing restlessness (unexpected/unstable) BEFORE patient with baseline anxiety
