Stridor

High-pitched inspiratory sound indicating upper airway obstruction

Definition

Stridor is a high-pitched, harsh, crowing sound heard during inspiration (or both phases in severe cases), caused by turbulent airflow through a narrowed upper airway.

Causes

  • Croup (pediatric)
  • Epiglottitis (EMERGENCY; do not examine throat)
  • Foreign body aspiration
  • Anaphylaxis with angioedema
  • Post-extubation laryngeal edema
  • Laryngospasm (post-thyroidectomy)
  • Tumor or abscess

Emergency Priority

Stridor is an AIRWAY EMERGENCY. Do not leave patient alone. Administer oxygen, call rapid response, prepare for intubation or tracheostomy, for anaphylaxis: epinephrine IM. For croup: racemic epinephrine, corticosteroids, for epiglottitis: keep child calm, NEVER examine throat or lie patient flat.
