Elasticity of skin, used to assess hydration
Definition
Turgor is the elasticity of skin, assessed by pinching the skin on the forearm, sternum (or abdomen in children) and observing how quickly it returns to normal.
Findings
- Normal: Skin returns to original position immediately.
- Poor turgor (tenting): Skin slowly returns, such as dehydration, malnutrition.
- Elderly: Diminished elasticity makes turgor less reliable; assess sternum or skin over clavicle instead.
Clinical Context
Use turgor alongside other dehydration indicators: dry mucous membranes, tachycardia, hypotension, oliguria, sunken fontanels (infants), elevated BUN/creatinine.
Nursing Interventions
Address dehydration: oral rehydration if tolerated, IV fluids if severe. Monitor vital signs, I&O, weight, electrolytes.
NCLEX Relevance
Tenting = dehydration cue. Forehead/sternum more accurate in elderly.
