Wheeze

High-pitched whistling sound from narrowed airways

Definition

A wheeze is a high-pitched whistling sound produced by air flowing through narrowed airways. It is typically heard on expiration but may be present on inspiration in severe obstruction.

Causes

  • Asthma (classic)
  • COPD
  • Bronchospasm (anaphylaxis, allergic reaction)
  • Foreign body aspiration (especially in children)
  • Pulmonary edema ('cardiac wheeze')
  • Tumor compression

Severity Indicators

Audible wheezing at rest = significant obstruction. SILENT CHEST in asthma exacerbation is an EMERGENCY. Not resolution but severe air trapping requiring immediate intervention.

Nursing Interventions

Administer short-acting bronchodilators (albuterol). Oxygen for hypoxia. Corticosteroids for inflammation. Monitor peak flow, SpO2, respiratory effort, for anaphylaxis with wheeze. IM epinephrine.
