Wheeze
High-pitched whistling sound from narrowed airways
Definition
A wheeze is a high-pitched whistling sound produced by air flowing through narrowed airways. It is typically heard on expiration but may be present on inspiration in severe obstruction.
Causes
- Asthma (classic)
- COPD
- Bronchospasm (anaphylaxis, allergic reaction)
- Foreign body aspiration (especially in children)
- Pulmonary edema ('cardiac wheeze')
- Tumor compression
Severity Indicators
Audible wheezing at rest = significant obstruction. SILENT CHEST in asthma exacerbation is an EMERGENCY. Not resolution but severe air trapping requiring immediate intervention.
Nursing Interventions
Administer short-acting bronchodilators (albuterol). Oxygen for hypoxia. Corticosteroids for inflammation. Monitor peak flow, SpO2, respiratory effort, for anaphylaxis with wheeze. IM epinephrine.
NCLEX Relevance
Silent chest in asthma = impending respiratory failure.
