Definition
Implementation is the fourth step of the nursing process (ADPIE), during which the nurse executes the planned interventions. It encompasses direct and indirect care, delegation, coordination, and education.
Components
- Reassess the patient before intervention
- Determine need for assistance
- Implement the intervention
- Supervise delegated care
- Communicate and document actions
Delegation Principles (Five Rights)
Right task, right circumstance, right person, right direction, right supervision. The RN retains accountability even when delegating. Cannot delegate assessment, planning, evaluation, or teaching to UAPs.
Nursing Considerations
Respect patient preferences, use evidence-based practice, consider safety and efficacy, document accurately and promptly.
