Rationale Scoring

Both action and reason must be correct for a point

Definition

Rationale Scoring is an NGN scoring rule used for dyad items (action paired with rationale). To earn credit, BOTH the chosen action AND the chosen reason must be correct.

Common Formats

Cloze (drop-down) items: 'The nurse should [action] because of [rationale].' Drag-and-drop dyads: match interventions to reasons.

Strategy

Identify correct rationale first. It often narrows the correct action. Conversely, selecting a correct action with wrong rationale earns zero. Think in pairs.
