Definition

The Highlight item format is an NGN question where the student clicks on specific words, phrases, or cells within a passage or medical record table to identify relevant or concerning cues. It simulates real-world EHR (Electronic Health Record) review.

Common Formats

  • Highlight text: A paragraph of clinical documentation where key findings must be clicked.
  • Highlight table: A lab report, vital signs log, or medication list where relevant data points must be selected.

Strategy

  • Read the full passage first to understand the clinical context.
  • Re-read to identify abnormal values, red flags, or trends.
  • Do not over-select: only click items that are clinically significant for the question asked.
  • Use partial-credit awareness: incorrect selections may cost points.
  • Consider age, diagnosis, medications, and history when determining what is concerning.

Clinical Judgment Mapping

Highlight items primarily test the Recognize Cues and Analyze Cues steps of the NCJMM. They evaluate whether the nurse can identify clinically relevant information from a complex data set.
