Measurable, observable information the nurse collects
Definition
Objective data are observable, measurable findings collected during assessment. They are verifiable by others and include vital signs, physical exam findings, lab results, and behavior.
Examples
- Blood pressure 142/88
- Temperature 38.7°C
- Patient grimacing while walking
- Clear amber urine
- Hemoglobin 11.2 g/dL
- Crackles in right lower lobe
Contrast With Subjective Data
Subjective data = what the patient reports (e.g., 'My chest hurts 7/10'). Objective data = what the nurse observes/measures.
Documentation
Document objective data clearly, quantifiably, and in measurable terms. Avoid subjective interpretations or judgments.
