Definition
Evaluate Outcomes is Step 6 of the NCJMM. The nurse compares the patient's response to the intervention with the expected outcome. Determining whether the plan was effective or needs modification.
Questions to Ask
- Did the intervention achieve the expected outcome?
- Is the patient improving, unchanged, or deteriorating?
- Are additional or alternative interventions needed?
- Should the plan be revised?
