Definition

Evaluate Outcomes is Step 6 of the NCJMM. The nurse compares the patient's response to the intervention with the expected outcome. Determining whether the plan was effective or needs modification.

Questions to Ask

Did the intervention achieve the expected outcome?
Is the patient improving, unchanged, or deteriorating?
Are additional or alternative interventions needed?
Should the plan be revised?
