Evaluate Outcomes

Step 6 of NCJMM: comparing actual to expected results

Definition

Evaluate Outcomes is Step 6 of the NCJMM. The nurse compares the patient's response to the intervention with the expected outcome. Determining whether the plan was effective or needs modification.

Questions to Ask

  • Did the intervention achieve the expected outcome?
  • Is the patient improving, unchanged, or deteriorating?
  • Are additional or alternative interventions needed?
  • Should the plan be revised?

Item Formats

Matrix (changes as 'improved' vs 'unchanged' vs 'worsened'), drop-down cloze, case study final questions.

Strategy

Apply systematic re-assessment. Outcomes tell you whether to continue, escalate, or change course.

NCLEX Relevance

Common as the final item in case studies. Tests full clinical judgment cycle.
