Subjective Data

Information reported by the patient (symptoms)

Definition

Subjective data are what the patient reports, such as symptoms, feelings, perceptions. They cannot be directly measured by the nurse.

Examples

  • 'My chest hurts 7/10.'
  • 'I feel nauseated.'
  • 'I have been having shortness of breath.'
  • 'I can't sleep at night.'
  • Pain rating, quality, location

Contrast With Objective Data

Objective data = observable and measurable (BP 140/90, temperature 38°C, crackles, RR 24). Subjective = reported by patient.

Documentation

Document subjective data using direct quotes when possible. Use 'patient states' or 'patient reports' to indicate subjective information.

NCLEX Relevance

Differentiate from objective data; both are essential to nursing assessment.
